Description:
· Ensures adequate home health clinical staff by recruiting, hiring, training
· Ensures that productivity and quality standards are met on a daily basis.
· Oversee and manage all clinical operations
· Oversee, evaluate, and manage all aspects of clinical services by monitoring clinical activities, including patient assessments, care plan development, care plan implementation , care plan coordination, care plan maintenance, care plan completion.
· Make RN home visits as needed to ensure that all referrals are accepted.
Minimum of 10 visits per week.
· Audit charts for clinical oversights and compliance and adherence to Medicare and state regulations
· Performs on-site filed visits with clinical staff to ensure clinical competency and adherence to home health regulations and company policies.
· Follow federal regulations and guidelines for Medicare reimbursement and Submit timely and accurate documentation
· Ensure service quality and participate in care coordination to ensure proper communication between clinicians, patients, physicians and referral source.
· Discuss operational issues, update staff on new/changed regulations and reviews records/documentation to ensure regulatory and in-house compliance
· Conduct and/or delegate the assessment and reassessment of patients, including updating of care plans and interpreting patient needs, while adhering to company, physician, and or/health facility policies and procedures
· Adheres to the policies and procedures of Santa Rita Home Health
· Adheres to Federal and State Regulations
· Communicate and demonstrate a professional image/attitude for patients, families, clients, coworkers and others
· Maintain licenses/certification and memberships in applicable professional organizations
· Coordinate communication between team members/attending physicians/caregivers/therapists to ensure the appropriateness of care and outcome planning and ensures compliance through review of documentation and care coordination activities
· Co-Manages our QAPI program by monitoring quality markers and developes improvement plans.
· Review and approve OASIS documents for accuracy and completeness, educate staff on proper OASIS documentation prior to locking and submitting data electronically.
· Explain and mentor clinicians and staff on billable skilled visits

· Manages Home Health infection control program which shall include monitoring, tracking and evaluation of infections.
Protects patients and employees by educating staff and adhering to infection-control policies and protocols.
· Maintains professional and technical knowledge by attending approved educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
· Complies with all federal, state, and local Home Health professional requirements
· Maintain relationships with referral/community sources
· Other duties as prescribed by the Home Health Administrator
Requirements:
Two years of Nursing experience with one working in Home Health
RN license- Active and in Good standing
Experience in working with adult and geriatric population
Good organizational, communication, interpersonal and managerial skills
Current CPR/Negative TB test/Current Fingerprint Clearance Card/Current Auto Liability Insurance
